In Spin-off: Major Grom. Monarch, 1–6 players work together to help Major Grom investigate a new threat in St. Petersburg. The game is a branching card-driven adventure set in the Major Grom film universe.

Players reveal cards according to their instructions. Some instructions must be resolved immediately, while other actions become available only when matching activation symbols have already appeared. Certain actions require spending money from the game's bank account, and upcoming event cards are triggered when enough matching symbols have accumulated. Player choices can remove cards from play and determine which paths remain available.

The game ends when the players reach either an achievement ending, which counts as a victory, or a "Case Closed" ending, which counts as a loss. The game contains twelve possible endings and can be reset and replayed to explore different outcomes.
